Release Forms
All Dickens Day of Writing participants must sign a photo/video/essay/multimedia release form. Students who are under the age of 18 must have a parent or guardian sign the form. Those over the age of 18 do not need an additional signature. Youth must come to the event with signed form(s) to participate. Students over 18 and mentors may sign the form(s) at the event.
SANTA CRUZ-SPECIFIC FORMS
MINORS: In addition to the Photo and Essay Release Form linked above, minors must also complete the following forms with their parent(s)/guardian(s):
- General Liability Waiver Form (PDF)
- Consent to Treatment of Minor (PDF)
- Emergency Contact Information for Minors (PDF)
ADULT MENTORS: In addition to the above linked Adult Photo Release Form, volunteer mentors must also complete the Volunteer Waiver and Election of Worker’s Compensation Form (PDF). Please also review the UC Santa Cruz Volunteer Pamphlet (PDF).
